Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Under the direction of the Manager, this position works as a member of the interdisciplinary team providing care within the context of a Patient/Client centered approach, the vision/values of Island Health and in accordance with BCCNM standards of practice, which reflect the core concepts and principles of Indigenous Cultural Safety, Cultural Humility and Anti-Racism. The Care Transitions Liaison Nurse assesses, organizes, implements and evaluates care plans for clients with chronic, acute, palliative or rehabilitative health care needs as they move between care settings. Duties include assessments, creation of care transition plans, coordination and disseminating information with client/family, community and tertiary acute care and rehabilitation services outside of the geographic region, care coordination, health teaching, support, consultation, liaison and referral while functioning as a member of a multidisciplinary team. The incumbent ensures that self care and independence is maintained, and participates in the administration of all acts and regulations applicable to nursing functions.

For positions with Indigenous Health, on average 80% of the Care Transitions Nurse's activities will be based in the hospital. For positions with Child Youth and Family,100% of the Care Transitions Nurse's activities will be based in the hospital.

QUALIFICATIONS: Education, Training And Experience

Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Must possess comprehensive knowledge of medical/surgical/gerontological nursing. Must be familiar with current Island Health acute, residential and community services.

  • Indigenous Health positions also require: Minimum of two years experience working with Indigenous populations in an acute or community setting, as well as demonstrated knowledge in Indigenous health approaches, protocol and cultural issues. Valid BC driver's license.
  • Child, Youth and Family positions also require: Comprehensive knowledge of Pediatric/Neonatal populations. Minimum of 2 years nursing experience working in acute care settings in Pediatric/neonatal including NICU or PICU.

  • Demonstrate discharge planning and care coordination skill sets.
  • Demonstrate ability to use the nursing process, i.e. assess (physical/mental/psychosocial/functional and environmental), analyze, plan, make decisions, organize, prioritize, evaluate, and communicate as required.
  • Demonstrate ability to prepare and maintain a variety of charts records and reports to work.
  • Ability to deliver culturally appropriate services to Indigenous populations.
  • Ability to operate related equipment including word processing, spreadsheet and database programs.
  • Knowledge of local Indigenous culture
  • Extensive knowledge of health issues impacting the Indigenous communities.
  • Broad knowledge of both Island Health and Indigenous services and community resources.
  • Computer skills, including the ability to use MS Office, knowledge of telehealth and other related computer technology;
  • Ability to carry out the physical demands of the job.
