What You’ll Be Doing
- Performs a person-centered health assessment of residents on admission, annually and as needed (including quarterly medication reviews) to develop a person-centered plan of care while collaborating with resident, family interdisciplinary team and other health professionals
- Evaluates the effectiveness of interventions and adjusts plan of care as needed in collaboration with resident or SDM, family, and/or interdisciplinary team.
- Performs appropriate screening and diagnostic investigations, interpreting results and assumes responsibility for follow-up.
- Accurately documents assessment plan, interventions and evaluations of resident care.
- Promotes knowledge development of clinical staff by integrating best practices in resident care while providing leadership and involves the interdisciplinary team in quality improvement initiatives.
- Provides proof of current registration with the College of Nurses as Registered Nurse in the Extended Class
- Has experience in Gerontology/Certification from the Canadian Nurses Association is an asset
- Is knowledgeable about primary health care issues in long-term care facilities.
