The Clinical Nurse Educator will provide leadership, education, and support to both healthcare staff and clients, ensuring high-quality, evidence-based care across various Lookout Society sites. This role includes developing and delivering educational programs for nurses, other healthcare staff and clients to improve health outcomes, promote preventive care, and enhance client engagement. The Clinical Nurse Educator will foster an environment of continuous learning for both staff and clients, supporting community health initiatives and promoting best practices in patient-centered care. Additionally, a weekly review of all care plans created by the nurse case manager will be conducted to ensure comprehensive documentation of all planned actions by the nurse.

What You Will Be Doing

Educational Program Development and Delivery for Lookout Society Staff and Clients:

  • Program Development and Evaluation: Develop, implement, and evaluate educational programs for Lookout Society staff and clients.
  • Training for Lookout Staff: Conduct training sessions, workshops, and seminars on clinical skills, care coordination, preventive care, and client engagement strategies. Create accessible educational materials for staff.
  • Client Education: Create accessible and culturally sensitive educational materials for clients on health topics, medication management, lifestyle modifications, and preventive care. Host client education sessions, workshops, and one-on-one consultations to improve understanding of their health conditions and empower them to take an active role in their care.
  • Client Engagement: Engage with clients to provide tailored education on managing chronic conditions, medication adherence, self-care, and healthy lifestyle choices, ensuring materials and strategies meet the diverse needs of the population served.


Clinical Competency Support

  • In collaboration with the Lookout Leadership Team, assess the educational and clinical skill needs of staff, to inform educational development.
  • Provide hands-on clinical support and mentorship to staff to help them build skills in client assessment, phlebotomy, and medication administration.
  • Stay current with developments in community healthcare and incorporate best practices into educational programs for both staff and clients.


Community Health Initiatives

  • Collaborate with community health and housing programs to align education with the specific health needs of the population, focusing on marginalized or underserved groups.
  • Educate staff on cultural competence, trauma-informed care, harm reduction, and community-specific health challenges.
  • Lead preventative care and wellness initiatives within the community.


Documentation And Reporting

  • Conduct weekly reviews of all care plans created by the nurse case manager to ensure complete documentation of all actions planned by the nurse.
  • Ensure accurate documentation of educational activities, competency assessments, and client education sessions.
  • Develop reports on training outcomes and identify trends in skill gaps and health literacy needs, making recommendations to improve training and clinical education practices.
  • Maintain records of all training and education activities for compliance with regulatory requirements.


Collaboration And Consultation

  • Act as a resource and consultant for clinical staff and patients, answering questions on procedures, health management, and preventive care.
  • Work with multidisciplinary teams to address client care challenges and improve delivery of care.
  • Participates as a member in the Medication Safety Committee and supports ongoing management of the Medication Manual.


About Your Qualifications

Education and Licensure

  • Registered Nurse (RN) with current and active licensure in good standing with the provincial regulatory body (e.g., BCCNM in British Columbia).
  • Bachelor of Science in Nursing (BSN) required.
  • Certified in CPR and First Aid; additional certifications in phlebotomy, harm reduction, or trauma-informed care are an asset.


Experience

  • Minimum 5 years of nursing experience, with at least 1 years in a leadership, educator, or clinical mentorship role.
  • Experience working in community health, public health, or primary care settings with marginalized or vulnerable populations is an asset.
  • Familiarity with harm reduction, trauma-informed care, and culturally safe care practices is an asset.


Knowledge, Skills, And Abilities

  • Strong knowledge of adult learning principles and instructional design for healthcare settings.
  • Excellent clinical skills with ability to coach and mentor nurses in areas such as client assessment, documentation, medication administration, and phlebotomy.
  • Ability to assess educational needs, develop materials, and evaluate training effectiveness.
  • Exceptional interpersonal and communication skills, with the ability to engage, motivate, and empower both staff and clients.
  • Able to work independently and collaboratively in a fast-paced, multi-disciplinary environment.
  • Strong organizational and time-management skills, with attention to detail in documentation and program evaluation.
