Qualifications

  • Anticipated location / home base is 7th Street Access Centre, Brandon
  • A baccalaureate degree in a health or social service discipline
  • Eligible for active practicing licensure to practice with applicable regulatory body in the province of Manitoba
  • Three (3) years of professional experience in a health care setting within the last five (5) years
  • Other combinations of suitable education and experience may be considered
  • Province of Manitoba Class 5 Drivers Licence, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health
  • Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
  • Demonstrated organizational skills and ability to work independently

Position Summary

The Service Navigator will provide first clinical contact with clients accessing Primary Health Care services. The Service Navigator will enable clients to have a positive, initial contact with the service delivery system at the Primary Health Care site. The Service Navigator plays an integral role on the team of service providers working out of the site, as they assess and meet client needs and, where applicable, direct them to the appropriate service provider and/or refer clients to other relevant community resources.

Responsibilities

Overview:

  • Conducts a thorough assessment of client’s needs, as well as assess their physical and emotional status upon entry to service.
  • Develops a thorough knowledge of health, education, and social services available.
  • Establishes effective working relationships with internal partners, as well as emergency shelters, community mental health and addictions providers, housing providers, and other community agencies and resources and helps individuals to navigate services.
  • Works with clients and other service providers to ensure services are not being duplicated.
  • Assist clients to determine service needs, engage them in service options, and refer to appropriate service provider/community resource for longer-term service needs, support clients as they follow through on accessing service.
  • Work collaboratively with interdisciplinary team to provide comprehensive services and supports to individuals.
  • Identifies when client needs fall outside the scope of the Primary Health Care site and refers them to the appropriate agency.
  • Treats each client and family as individuals and help them to problem solve, using a strength based, client centered approach.
  • Maintains flexibility, adaptability, and non-judgmental approach when assisting clients in the decision-making and problem-solving process.
  • Works with clients to empower them to take control of their own health and life situations.
  • Links with potential clients out in the community to make them aware of the services available (outreach).
  • Assists clients with accessing amenities and basic needs available on site and in the community.
  • Works with clients and service providers to advocate on behalf of client needs.
  • Applies a harm reduction philosophy and population health principles to all aspects of the position.
  • Assists clients in skill development that will optimize success related to the determinants of health such as job searches, problem solving, coping skills, preparing for employment and other types of interviews, coaching individuals on how to be successful in maintaining employment, and on how to access other services successfully.
  • Administers the Brandon Ministerial Benevolent Fund, according to guidelines and as funds are available.
  • Oversees client access to voicemail program available at the Primary Health Care site.
  • Coordinates client access to other site amenities.
  • Promotes all services and programs to clients, service providers, and the community.
  • Works with service providers within the Primary Health Care setting and the community to coordinate and promote education programming for the public.
  • Provides back-up support to reception as needed, to assist with client needs and ensures respectful behavior is displayed at all times.
  • Monitors client behaviors and addresses/reports inappropriate behaviors immediately – ensures respectful behaviors are reinforced.
  • Notifies manager of all risk situations.
  • Other duties as assigned.
