Description

POSITION TITLE: Supportive Living Coordinator

PROGRAM: Operations – Supportive Living (Group Homes)

POSITION TYPE: Part-time, fixed-term contract through March 31, 2026

HOURS OF WORK: 28.5 hours per week; Thursday – Saturday; 1:00pm – 10:30pm

POSITION SUMMARY: The Coordinator plays a crucial role as the first point of contact, providing information and support as individuals embark on their wellness journey. This role upholds a high standard of care while offering compassionate service, problem-solving, and responsive support to meet client needs.

REPORTS TO: Team Lead, Supportive Living

Key Service Responsibilities And Specific Accountabilities

  • Collaborate with clients and their support teams to develop and regularly review Service Plans with measurable goals. Conduct follow-ups to support adherence and address emerging concerns.
  • Liaise with community partners on a regular basis to ensure collaborative support in helping clients achieve their personal goals.
  • Assist clients in establishing and maintaining peer, family, and community support networks.
  • Provide consultation and supportive counselling during emergent or crisis situations.
  • Advocate for clients to ensure they receive adequate care, treatment, and information from the community and CMHA.
  • Facilitate a cooperative living environment through regularly scheduled community/house meetings and individual interventions as needed.
  • Maintain effective communication and documentation systems to support client progress, while adhering to communication protocols.
  • Perform regular rounds and perimeter checks as instructed.
  • Perform cleaning and sanitation duties according to the shift rotation.
  • Support and maintain a safe environment for participants and staff by performing duties that promote physical, emotional, and psychological safety within the housing setting.
  • Administer lifesaving interventions such as CPR/First Aid, Naloxone, etc., when necessary.
  • Ensure accurate and up-to-date client records are maintained in accordance with agency and privacy legislation.
  • Other duties as required.

Qualifications And Skills

  • Bachelor of Social Work (BSW), Bachelor of Psychology, or a recognized diploma or undergraduate degree in a human services field. Equivalent experience may be considered.
  • At least two (2) years of experience working with persons with a diagnosed mental health condition, preferably in a skill development or residential environment.
  • Experience working with individuals with substance abuse issues.
  • You have worked in the mental health field for over two years and have a deep commitment to and understanding of the recovery model.
  • Current Applied Suicide Intervention Skills (ASIST) certification.
  • Current First Aid and CPR certifications.
  • Must be able to work flexible shifts.
