Physiotherapist – Community Advantage Rehabilitation (CAR) – a Division Of Lifemark

Location: Pickering - Durham Region, ON

Sign-on bonus up to $5000.00 available!

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Physiotherapiststo provide services to our pediatric/young adult clients residing in group homes within the community. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size.

PT Responsibilities:

  • Provide ministry funded, community based Physiotherapy services within the pediatric/young adults client’s place of residence and/or through virtual services
  • Provide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of gross motor skills, co-ordination, neurological, developmental, mobility aids, etc.
  • Develop and implement goal-oriented treatment plans in collaboration with clients/caregivers
  • Effectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriate
  • Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards
  • Opportunity to service private clients


Requirements:

  • Registered Physiotherapist with the College of Physiotherapists of Ontario (required)
  • Experience working within a community setting is an asset but new graduates are welcome
  • Ability to work independently and as part of an inter-professional team
  • Ability to prioritize, manage self-scheduling and meet reporting deadlines
  • Excellent oral, written and report writing skills
  • Proficiency with computers
  • Strong interpersonal and organizational skills
  • Reliable transportation is required for this position
