Position Overview:
We are looking for one temporary (12-month) full-time Proactive Care Coordination Assistant (PCCA)! The PCCA will work with multiple PCN clinics on a rotating basis. The PCCA with work with doctors, office staff, and clinical team members, and combination of in-person/clinic and remote work may be required. The PCCA will work with the clinic’s electronic medical record (EMR) to track patients and their care. Following a PCN-directed plan, the PCCA will assist the clinic in identifying and contacting patients due for preventative health screening (e.g., cancer screening such as mammograms) and chronic disease management appointments. This may involve performing chart reviews or running clinic level reports.
The PCCA will work as part of the Quality Improvement team and report to the Proactive Care Coordination Lead.
Key Roles and Responsibilities:
- Identify and contact patients due for care and book appointments with physicians and multidisciplinary team members.
- Update EMR record of patients’ demographic information, and the status of those no longer on physician panel
- Update EMR record with external preventative health screening results from provincial health record. Contact patients due for preventative health screening to offer screening requisitions and book appointments.
- Update EMR record and contact patients in relation to quality improvement initiatives directed by PCN QI Team.
- Track and report data from the EMR
Experience, Skills & Qualifications:
- Medical Office Assistant/Unit Clerk certification
- Medical terminology course
- Reliable transportation
- Excellent communication skills
- Strong time management and organizational skills
- Able to balance multiple priorities and problem solve
- Excellent computer knowledge, including Excel
- High attention to detail
- To support the vision and mission of the Primary Care Network
- Previous experience:
- Family medicine clinic/Primary care setting
- Using Electronic Medical Records
