Job Overview

We are pleased to invite applications to join the Breast Clinical Nurse Specialist (CNS) team based at Parapet, Windsor.

More people are living with and beyond cancer. Receiving care that is tailored to a person’s needs can have a significant impact on their experience and quality of life. Providing personalised stratified follow-up is a vital part of delivering world class cancer services.

Supported by the Band 8a Cross-Site CNS Team lead, the successful post holder will work together with cross-site colleagues to establish a dedicated team to drive forward the Trust’s established Supported-Self-Management-Pathway (SSMP) service, also known as Patient Initiated Follow-Up (PIFU), in line with national cancer strategy to deliver personalised care.

The CNS plays a pivotal role in ensuring excellent experience of care for breast cancer patients, both during and beyond their treatment. There is an expectation that the post holder will also provide ad hoc assistance to the Primary CNS team, in supporting patients newly diagnosed with a primary breast cancer and throughout their treatment. The post holder with gain experience in all aspects of care co-ordination and service delivery in the active treatment phase, as well as lead in the transition to supported-self-management and beyond.

The role will be primarily based at The Parapet, Windsor, with a cross-site overview.

Main duties of the job

Establishing a dedicated team, the successful applicant will have the responsibility for the ongoing development and delivery of a comprehensive nurse-led service that is holistic in its approach, evidence-based and meets the individual needs of breast cancer patients, following their active treatment. The post requires the ability to work autonomously, as well as within the team.

We are looking for a forward-thinking senior nurse who is highly motivated to develop quality improvement opportunities within personalised care for breast cancer patients. The successful candidate will be ambitious and dynamic in their approach to service transformation and innovation.

The role will require care co-ordination with the MDT; delivering nurse-led clinics; providing specialist advice on breast awareness, including information about signs and symptoms to look out for, which could suggest their cancer has returned; and facilitating rapid re-access to their breast cancer team, including telephone advice and support, if they are worried about any symptoms, including side effects of treatment.

There will be an opportunity to work with the Macmillan Information and Support Service to develop established Health and Well-Being Days and other exciting initiatives. Administrative support will also be provided.

Essential criteria

  • Degree in Nursing and current NMC registration
  • Breast/oncology/palliative care qualification
  • Leadership / project management qualification
  • Teaching or Mentorship qualification

Desirable criteria

  • Advanced communication skills training / Sage and Thyme training
  • Psychology Level 2 training
  • Physical assessment qualification
  • NMP qualification
  • Prosthesis fitting qualification

Experience

Essential criteria

  • Management / leadership experience
  • Band 6 CNS experience
  • Experience delivering nurse-led clinics
  • Breast cancer / oncology / SACT / palliative care experience
  • Experience in MDT working

Desirable criteria

  • Band 7 CNS experience
  • Team Management experience

Skills and Knowledge

Essential criteria

  • Awareness of national cancer strategy and guidelines
  • Previous patient education / support / counselling skills
  • Evidence of continuous professional development.
