CORE RESPONSIBILITIES OF THE POSITION

This position will support the mission, vision and values of the Windsor Essex Ontario Health Team (WEOHT), Windsor Essex Community Health Centre (weCHC) and the work of the Refugee Health – Coordinated Access Program and support safety, quality care of clients/patients. The Clinical Care Coordinator, RN will:

• Act as a resource to and advocate for Refugees (individuals, groups, and the community)
• Establish and maintain partnerships and professional relationships with internal teams and community-based organizations to support a coordinated access pathway and optimal continuum of care
• Participate in program development, adaptation, implementation, monitoring, and evaluation, including education, counselling, and health promotion services
• Protect the confidentiality, privacy, and security of client and corporate information
• Support the inter-professional team with program areas, supplies, and equipment, ensuring safe storage and maintenance
• Field calls for Refugee Health program; schedule clients with appropriate teams/providers; navigate weCHC and external services based on client needs
• Perform physical assessments, health education, counselling, care, and treatment per College of Nurses of Ontario standards, scope of practice, provider consultation, and/or medical directives
• Review client referrals to determine appropriateness for weCHC services; assist with related paperwork (applications, appointments, etc.)
• Work collaboratively with primary care providers and other health care professionals, involving clients in all aspects of their treatment and care
• Contribute to chart documentation, chart reviews, case conferences, and progress reports to referring/non-referring providers
• Participate as an inter-professional team member in planning, team meetings, and related functions
• Care for complex clients whose outcomes may be unpredictable
• Perform other duties supporting the organization's mission and vision, as assigned

SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED

• Minimum of 3-5 years of nursing experience in a community setting or combination of community and hospital or public health settings
• Experience working with Refugees and surrounding community.
• Knowledge of policies, legislations, programs and issues related to area of practice
• Demonstrated ability to work with sensitivity and compassion to deal with vulnerable populations.
• In-depth understanding of community resources and programs.
• Strong commitment to health prevention and client education programs to support clients throughout their lifespan
• Knowledge of health care services and system navigation
• Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
• Experience in program development, implementation, monitoring and evaluation
• Able to convey complex health concepts to the lay public
• Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation to support travel across Windsor-Essex County and the province, as required

Education and certificate requirements

• Master of Science in Nursing (MScN) from a recognized University
• Member in good standing and current registration with the College of Nurses of Ontario (CNO)
• Completion of a police clearance and vulnerable sector check prior to commencing employment
• Current Basic Rescuer (C) certificate recognized by the Heart and Stroke Foundation of Ontario
