Camrose Home Care is seeking an experienced Registered Nurse for its Home Care team. Reporting to the Care Manager, Camrose Home Care in the role of Case Manager, you will be responsible for coordinating and providing quality care to clients in the community. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning. Through the development of the care plan you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates and be on call. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications

Active registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Graduation from a recognized nursing education program. Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel. Driver's Abstract to be completed by AHS with no GDL restrictions. In addition, the incumbent will be expected to demonstrate proficiency in the following areas: Actively integrates Patient and Family Centered Care principles into practice. Excellent communication and interpersonal skills in order to work collaboratively with members of the multidisciplinary team.

Preferred Qualifications

Minimum one (1) years community nursing experience. Experience with Meditech, Care Manager, Microsoft Office and general computer skills. RAI-HC training. Demonstrates critical thinking, conflict resolution and problem solving skills. Excellent time management and prioritization skills. Competence in relevant technical nursing skills and a willingness to learn new skills as required.
