As one of a network of provincial community health centres, Bridges CHC is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and illness prevention programs and services.
AS A MEMBER OF OUR INTERDISCIPLINARY TEAM, THE NURSE PRACTITIONER:
• functions independently but works alongside other Centre staff to provide client-centered care that emphasizes health promotion, disease prevention/treatment and client education.
• The nurse practitioner provides primary health care, within the PHC NP scope of practice, to clients at all stages of their lives and with all health conditions.
• The nurse practitioner practices according to the Standards of Practice of the College of Nurses of Ontario (CNO) and guidelines and policies established at Bridges Community Health Centre.
• The nurse practitioner works in partnership with other staff and management to support the Vision, Mission, and Values of the Centre.
• Qualified to register or be registered in good standing with the College of Nurses of Ontario as a Registered Nurse in Extended Class.
• Bachelor or Master of Science in Nursing from a recognized university.
• Three to five years’ experience in a health care setting with preferred experience in the community.
• Demonstrated ability, commitment to and knowledge of community health care, the social determinants of health and health promotion.
• Demonstrated commitment to equity, diversity, inclusion, and anti-racism with a willingness to engage in ongoing learning to increase self-awareness, improve practices and eliminate disparities in health status.
• Awareness of the importance of, and experience with, providing trauma-informed care.
• Excellent interpersonal skills with clients in a culturally diverse practice population, and with colleagues.
• Demonstrated ability to work effectively in an interdisciplinary team environment.
• Excellent communication, decision-making, problem-solving, conflict management and time management skills.
• The ability to work at either Bridges CHC location and work flexible hours.
• Participation in our afterhours, on-call rotation schedule.
