Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. The Health Care Aide provides personal care and support to clients in a community setting. Work is assigned according to an established plan of care, in collaboration with the client, family and members of the multidisciplinary team. The Health Care Aide is supervised by a regulated health professional and may perform assigned restricted activities under specific circumstances. Documentation of pertinent information and care provision is required, as is participation in relevant in-service education. The Health Care Aide provides patient care services as an unregulated worker under the supervision of a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), or Licensed Practical Nurse (LPN). This position will be based out of the Westview location office but will provide support to home care's Suburban/Rural West team, which will include travel to the other home care network offices, as required. As a Health Care Aide (HCA), you will work as a collaborative member of the multidisciplinary team, providing personal assistance by supporting activities of daily living and delivering support services to patients (clients, residents) who require short term assistance or ongoing support, in accordance with the patient care plan and facility policies and procedures. In this role, you will work closely with patients, families, and caregivers across the continuum of care in home, community, and health care facilities. You will assist nurses with the provision of routine care to meet patients' hygiene, nutrition, mobility, recreation, and safety needs; assist with organized activities or programs; and observe and report on patients' mood, health, and wellbeing. Working under the supervision of a regulated health professional, you will assist in maintaining a safe environment for patients, staff, and visitors, while supporting effective communication with patients and health care professionals. PLEASE NOTE: Until a new hire provides proof of HCA certification, they will be paid at the uncertified pay grade in accordance with the AUPE AUX Collective Agreement.

Required Qualifications
Active or eligible for registration or practice permit with the College of LPNs and HCAs of Alberta (CLHA) as a Health Care Aide, along with proof of education or completion of Health Care Aide Certificate within 18 months of hire.
Additional Required Qualifications
Basic Cardiac Life Support- Healthcare Provider (BCLS HCP). Communication and assessment skills. The Ability to apply health and safety principles in delivering care to clients in the community, to work effectively and cooperatively with others and to set priorities by organizing and scheduling work-related activities is essential. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.
Preferred Qualifications
Previous experience in the health care field and work with older or chronically ill individuals. A Medication Assistance Certificate or prior experience in Medication Assistance.
