- Assess and treat clients with a wide range of acute and chronic conditions
- Restore movement and function through exercise, strength training, mobility retraining, and balance programs
- Support rehab needs, including musculoskeletal, neurological, pulmonary, and cardiovascular care
- Develop personalized care plans that reflect client goals and promote independence
- Measure and track progress using evidence-based, objective tools
- Educate and empower clients and their families to build confidence in self-care and long-term health
- Prescribe and train in the use of mobility aids, adaptive devices, transfer aids, and rehab equipment
- Collaborate with case managers, caregivers, and interdisciplinary team members to deliver client-centered care
- Refer and connect clients with appropriate community resources and support
- Registration (in good standing) with the College of Physiotherapists of Ontario
- Strong clinical expertise with a focus on rehabilitation and functional recovery
- Excellent communication and teaching skills to inspire clients and families
- Strong critical thinking, problem-solving, and sound decision-making
- A desire to promote autonomy for our clients and families to live independently
- Ability to work both independently and as part of a dynamic care team
- Valid driver’s license (G2 or higher) and reliable vehicle
- 2-5 years of clinical practice experience preferred
- ADP Authorizer status an asset
- Community health and/or home care experience an asset
- Bilingualism (French/English) considered an asset
