ABOUT THE ROLE

The LPN – Assistant Unit Supervisor plays a central role in delivering safe, compassionate, and resident-centred care at The Meadows. Working within your scope of practice, you will provide direct nursing care to medically stable residents while leading, coaching, and supervising CCA, Floor LPN, SSW, FSW, and ESW team members across your assigned households.

This is a leadership position for an LPN who is ready to take ownership of household operations, drive quality care outcomes, and contribute to a positive workplace culture rooted in the Eden Alternative philosophy.

KEY RESPONSIBILITIES

Resident Care (80% of role)

  • Perform assessments and reassessments; use tools such as MMSE, Scott Fall Risk, and Braden Scale
  • Administer medications, treatments, glucometers, vitals, and wound care within LPN scope
  • Develop, revise, and evaluate resident care plans in collaboration with the interdisciplinary team
  • Round on assigned households each shift; ensure care aligns with the Care Plan and Kardex
  • Complete narcotic counts, lab requisitions, specimen collection, and medication cupboard management
  • Communicate medical status changes to the RN Supervisor and collaborate on unstable residents
  • Prepare and send end-of-shift reports to DOC, ADOC, Clinical Coordinator, and HR Manager (Charge Nurse shifts)
  • Observe mealtimes for correct texture/diet; collaborate with dietitian on nutritional concerns
  • Use PIECES tools for responsive behaviors; engage residents in meaningful daily activities

Supervisory Responsibilities

  • Monitor, coach, and supervise CCA, Floor LPN, FSW, ESW, and SSW performance
  • Conduct initial investigations of staff incidents and behavioral concerns; prepare reports for management
  • Intervene in serious misconduct situations (including placing employees on administrative leave after hours)
  • Participate in hiring, orientation, and informal performance feedback for nursing and care staff
  • Serve as a liaison on interdepartmental issues and resident/family concerns

Operations & Compliance

  • Ensure compliance with provincial and federal regulations (locked medications, privacy, OHS)
  • Complete incident reports and initiate investigations in a timely manner
  • Participate in multidisciplinary care meetings, pharmacy committee, and quality indicator data collection
  • Order medical supplies; maintain organized charts, doctor’s orders, and standing admission orders

QUALIFICATIONS

Required

  • Current registration as an LPN with the Nova Scotia College of Nursing (NSCN)
  • College Diploma (2 years) in Practical Nursing
  • Completed Pharmacology through the LPN Core Program or an approved post-graduate course
  • Minimum 2 years nursing experience demonstrating judgment, leadership, and clinical competence
  • Minimum 1 year recent experience in medication administration
  • Current or willingness to obtain CPR certification and Alzheimer’s Disease & Related Dementia Care certification

Preferred

  • Previous supervisory or charge nurse experience
  • Experience working with older adults, including those living with dementia
  • Familiarity with Eden Alternative and resident-centred care philosophies
  • Knowledge of WHMIS, fire safety, evacuation procedures, and OHS legislation
