Registered Practical Nurse

Elevate NWO is seeking a Registered Practical Nurse (RPN) to support nursing care, health education, testing, treatment support, clinical follow-up, and community-based outreach across our clinic, Drop-In Centre, outreach, and community settings.

The RPN reports to the Director of Community Health Services and works within CNO standards, RPN scope of practice, organizational policies, and applicable clinical directives and protocols.

This role supports accessible, low-barrier care for people living with or at risk of HIV, Hepatitis C, STBBIs, substance use, housing insecurity, homelessness, poverty, stigma, and related health or social barriers.

Key Responsibilities

• Provide nursing care, health education, testing, treatment support, and follow-up

• Support patient assessment, care planning, treatment monitoring, adherence support, and follow-up

• Conduct point-of-care and laboratory-based testing for HIV, HCV, STBBIs, and related health concerns

• Perform venipuncture, specimen collection, and other clinical procedures within RPN scope

• Provide pre- and post-test counselling, referrals, and harm reduction support

• Maintain testing and clinical supply inventory and support exam room readiness

• Participate in outreach, community-based testing, health promotion, and education activities

• Document patient encounters, testing, referrals, follow-up, and clinical care in required systems

• Work collaboratively with internal staff, health-care providers, and community partners

Qualifications

• Registered Practical Nurse with current registration in good standing with the College of Nurses of Ontario

• Two to three years of relevant nursing experience preferred

• Experience in community health, primary care, harm reduction, outreach, infectious disease, or related settings is an asset

• Experience with point-of-care testing, phlebotomy, specimen collection, or related clinical procedures

• Knowledge of HIV, HCV, STBBIs, testing, prevention, treatment support, harm reduction, and substance use

• Strong communication, clinical judgment, adaptability, initiative, and documentation skills

• Ability to work respectfully with people experiencing stigma, poverty, homelessness, housing insecurity, substance use, and related barriers

• Commitment to harm reduction, health equity, trauma-informed care, and anti-stigma practice

• Ability to pass a Vulnerable Persons Criminal Reference Check

• Valid Class G Ontario driver’s licence and willingness to provide a clean driver’s abstract required

Compensation

Year 1 on the Registered Practical Nurse wage grid is currently $29.89 per hour. This is a unionized position and compensation/benefits are administered in accordance with the Collective Agreement.
