To provide direct professional nursing care for patients requiring complex mental health services. Utilizing the principles of the nursing process, the RN will implement autonomous nursing actions to ensure that services provided promote positive patient outcomes. The RN, in partnership with the patients and their families and in collaboration with the clinical team, will coordinate and support the patient’s journey toward maximum independence and wellness.

Skills And Qualifications Required

  • Registered Nursing diploma; Degree in Nursing preferred
  • Current unrestricted registration with the College of Nurses of Ontario
  • Minimum of three years of work experience providing recovery focused services to adults with a serious mental illness, within an inter-professional team
  • Minimum of one-year of work experience providing services to adults with a serious mental illness, effectively accessing and employing community resources to meet the needs of the individual.
  • Recent forensic experience preferred

Position Responsibilities

  • Develops effective therapeutic relationships including developing shared understanding of the individual's lived experience with the illness, assertively reaching out to clients/care providers in their natural environments
  • Provides service coordination for assigned individuals including coordinating, monitoring and revising the delivery of treatment, rehabilitation and support services per the individual's recovery plan and with a focus on individual strengths
  • In collaboration with the team psychiatrist and other nurses on the team:
    • Develops, revises, maintains, and leads team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering including by injection, evaluating, and recording medications prescribed by the team psychiatrist
    • Evaluates and charts medication effectiveness, complications, and side effects; arranges for required lab work according to protocol
    • Organizes and manages the system of getting medication to clients and integrating medication administration within the client's recovery plan
    • Manages pharmaceuticals and medical supplies
  • Provides ongoing assessment, problem-solving, side-by-side services, skill training, supervision (e.g. prompts, assignments, monitoring, encouragement), and environmental adaptations to assist clients with activities of daily living
  • Assists individuals to develop lasting connections to communities and natural supports by providing individual supportive therapy (e.g. problem-solving, role­ playing, modeling and support), social-skill development, and assertiveness training to increase the individual's social and interpersonal activities in community settings
