The Canadian Mental Health Association, Simcoe County Branch seeks an RPN, who, in accordance with the agency’s policies, standards, and guidelines, provides client-centered assessment, treatment, recovery, and rehabilitation/support services based on guidelines of the ACTT model of care.

What You'll Do:

Deliver nursing care within the RPN scope and in accordance with the College of Nurses of Ontario standards.

Review and transcribe physician orders, monitor lab results (including CSAN for clozapine), and manage medication administration and reconciliation.

Conduct physical and mental health assessments, monitor symptoms and medication effects, and report changes in client status.

Provide counselling, therapeutic interventions, and support for daily stressors and recovery goals.

Collaborative, Multidisciplinary Care:

Work as part of a multidisciplinary ACTT team, participating in client-centered, recovery-oriented service delivery.

Coordinate individualized recovery plans and consult with other providers to ensure continuity of care.

Deliver the majority of services in community settings, including client homes and public spaces.

Provide mobile outreach, transportation, and support for clients within and occasionally outside the geographic area.

Assist clients with activities of daily living (ADLs), housing support, and life skills development.

Educate clients, families, and community stakeholders on mental health, healthy lifestyles, and ACTT services.

Participate in daily team reviews, risk management, and care planning.

Provide on-call crisis intervention and contribute to the ongoing development of ACTT protocols and services.

Minimum three (3) years experience in psychiatric nursing (community-based preferred).

A Registered Practical Nurse registered as a member in good standing with the College of Nurses of Ontario.

Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.

Submission of a Vulnerable Sector Check (VSC), including a Criminal Record Check (CRC) in good standing.

Fluency in French or another second language would be an asset.

Legally entitled to work in Canada

RPN: $56,420 - $62,226 *per annum

*prorated based on hours worked

*plus shift premium as per policy

Thirty-five (35) hours per week.

Rotational shift work based on a seven (7) day staffing schedule.

Program provides service 365 days per year.

Able to work within a flexible framework to respond to client needs.
