We need a Registered Practical Nurse to join our Transitions in Care Team! As the successful applicant, you will do the following:

  • Contributes within the scope of an RPN as a team member in the assessment, treatment, provision of care and health promotion for all patients through supportive, therapeutic, palliative and rehabilitative means while adhering to the standards mandated by the College of Nurses of Ontario.
  • Assist in the care of admitted patients and those patients that are pending discharge
  • Perform admission assessments and patient care required related to admission interventions
  • Communicate thorough and systematic transfer of accountability (TOA)
  • Document admission interventions and TOA
  • Become fluent in P4R metrics, collect simple statistics
  • Provide feedback and improvement ideas

What You’ll Bring

Required

  • Current RPN Registration with the College of Nurses Ontario
  • Minimum of one-year recent experience in Acute Nursing Care
