Inaugural Director of Care: Lead with Heart and Purpose

Neshama Hospice | Toronto, Ontario

Full-Time | On-Site

Build the Team. Shape the Culture. Open the Doors.

Neshama Hospice is seeking a compassionate and operationally driven nurse-leader to serve as our Inaugural Director of Care as we prepare to open Canada’s first hospice residence built on Jewish values, open to all. Currently under construction in North York and on track to open in late 2026, we will welcome approximately 250 residents and families annually into a home-like environment where dignity, individualized care, and holistic support define every moment.

Working closely with the Medical Director, you will ensure clinical excellence is matched by operational reliability—bringing the care team, systems, and culture together to deliver an exceptional experience for every resident and family.

An Extraordinary Opportunity

• Design the nursing and care delivery model that defines how compassionate, person-centred hospice care is delivered day-to-day

• Lead the development of clinical policies, workflows, staffing frameworks, and IPAC protocols before our doors open

• Build and mentor an interdisciplinary care team—RNs, RPNs, PSWs, and allied health—who will be the heart of Neshama’s care

• Shape a culture where collaboration drives better outcomes and every decision honours the whole person: body, mind, spirit, and community.

About Neshama Hospice

Neshama means “soul” or “spirit,” reflecting our belief that dying encompasses far more than the physical. Founded on Jewish values of loving kindness, repairing the world, and caring for the sick, we will welcome everyone into a 12-bed community built on compassion, respect, and dignity.

Neshama will be a place where residents and families are partners in care; where excellence means addressing pain, suffering and healing across all dimensions— physical, emotional, spiritual, and social; where a collaborative team model ensures that wisdom flows from all directions, and where care is personalized, holistic and inclusive.

The Role

Reporting to the Executive Director and working in close partnership with the Medical Director, you will:

• Oversee day-to-day delivery of safe, high-quality hospice palliative care aligned with person- and familycentred values

• Build and sustain a 24/7 clinical staffing model—recruiting, onboarding, scheduling, coaching, and leading the care team

• Direct the admissions process and resident flow, ensuring timely and well-coordinated access to care

• Lead clinical quality, risk management, IPAC, and accreditation readiness

• Partner with the Executive Director on operational planning, workforce strategy, and budget management

• Foster interdisciplinary teamwork, team culture, and continuous learning across all clinical and support roles

• Collaborate with Ontario Health at Home, referral partners, and community and clinical stakeholders

This is an inaugural role—a chance to leave a legacy that will influence thousands of lives and redefine approaches end-of-life care.

Who You Are

• Minimum 3 years’ experience in healthcare leadership, including direct staff supervision

• Minimum 3 years’ clinical experience in hospice palliative care or a closely related setting

• Certification in palliative care (e.g., CAPCE) or equivalent preferred; Health and Safety Certification

strongly preferred

• Proven experience managing staffing, scheduling, and clinical operations in a 24/7 care environment

• Strong knowledge of IPAC, quality improvement, and clinical risk management

• Demonstrated ability to lead interdisciplinary teams and collaborate with community partners

• Proficiency with EMR systems and scheduling software

• Valid Driver’s Licence, access to a vehicle, and satisfactory Vulnerable Sector Screening required

This is Not a Typical Director of Care Role

This is for a nurse-leader who wants to build—from the ground up—a clinical culture, team, and model that will shape what exceptional end-of-life care looks like in a first-of-its-kind hospice in Canada. You will lead the development of clinical infrastructure, EMR selection, workflows, hiring frameworks, and accreditation readiness before the residence opens. If you believe how we care for people at the end of life is among the most important work we can do—and you want to lead it from day one—this is your calling.
