The Elliot Lake Family Health Team (ELFHT) is seeking a compassionate, collaborative, and motivated Social Worker to join our growing interdisciplinary team.
At ELFHT, we provide comprehensive primary health care services including chronic disease management, health promotion, disease prevention, and mental health support to both rostered and unattached patients in Elliot Lake and surrounding communities. Our team-based model of care supports innovation, collaboration, and patient-centered service delivery.

About the Role
The Social Worker will provide mental health assessment, treatment, and short-term counselling services to patients with a wide range of presenting concerns. Working both independently and collaboratively within an interdisciplinary primary care setting, the successful candidate will:
- Maintain a clinical caseload providing evidence-based mental health care
- Triage physician and interdisciplinary referrals to the FHT Mental Health Program
- Support patients in accessing appropriate community and social services
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary health professionals to develop comprehensive care plans
- Participate in community partnerships and initiatives aimed at improving mental health supports within Elliot Lake and the surrounding area
- Contribute to a positive, patient-focused, team-oriented clinical environment
Qualifications
· Master’s Degree in Social Work (MSW)
· Registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers
· Valid professional liability insurance
· Minimum 2–3 years of clinical experience, preferably in primary care or community mental health
Preferred Skills & Experience
- Strong clinical assessment, treatment, and case management skills
- Experience supporting mental health concerns commonly seen in primary care settings
- Knowledge of CBT, DBT, and ACT therapeutic approaches
- Experience facilitating groups and psychoeducational programming
- Strong systems navigation skills and familiarity with community resources
- Knowledge of the Mental Health Act and related legislation
- Experience working with older adults/geriatric populations is considered an asset
- Familiarity with electronic medical records (EMR)
- Knowledge of privacy legislation and PHIPA requirements
- Bilingualism (English/French) is considered an asset
