The Elliot Lake Family Health Team (ELFHT) is seeking a collaborative and patient-focused Clinical Pharmacist to join our interdisciplinary primary care team.

At the ELFHT, we provide comprehensive primary health care services including chronic disease management, health promotion, disease prevention, and mental health support to rostered and unattached patients in Elliot Lake and surrounding communities. Our team-based model of care supports innovation, collaboration, and patient-centred practice.

About the Role

The Clinical Pharmacist is an integral member of the ELFHT interdisciplinary team and plays a key role in supporting safe, evidence-based, and patient-centred medication management.

Working collaboratively with physicians, nurse practitioners, nurses, and other interdisciplinary health professionals, the successful candidate will support patients with complex medication needs, chronic disease management, preventative care, and health education initiatives.

Key Responsibilities

· Provides direct patient care including comprehensive medication reviews, chronic disease management, and minor ailments assessments

· Identifies and resolves drug therapy problems independently and in collaboration with the interdisciplinary team

· Provides clinical consults and drug information services to the physicians and interdisciplinary health professionals in accordance with current evidence-based practice

· Supports the safe use of medications through delivery of medication monitoring programs (e.g., INR monitoring)

· Documents all patient encounters and consults in the electronic medical record (EMR) according to the standards of the Ontario College of Pharmacists

· Provides education to patients and the interdisciplinary team to support the safe and effective use of medications

· Conducts medication reconciliation, identifying and resolving any discrepancies to ensure the accurate recording of patient medication information in the EMR

· Supports the development of policies and procedures to optimize drug prescribing, handling of drug samples, administration of medications/immunizations, etc.

· Presents on specific therapeutic topics as requested to educate the interdisciplinary team on new drug therapies, updates to clinical practice guidelines, or to highlight medication safety issues

· Participates in the clinical instruction of medical learners and other health professional students as required

· Attends monthly meetings of the IHPs to share updates on pharmacist services and programming

· Communicates with hospitals, community pharmacies, and other external organizations as required to ensure smooth transitions of care between sites

· Works collaboratively and respectfully with other professionals on the team, support staff, and patients

Qualifications

· Bachelor of Science in Pharmacy or a Doctor of Pharmacy degree from an accredited university

· Current registration with the Ontario College of Pharmacists (Part A) and in good standing

· Maintenance of professional liability insurance

· Demonstrated problem-solving and organization skills with sound clinical judgement

· Excellent verbal and written communication skills

· Experience using EMRs to document patient care, support program delivery, and quality improvement initiatives

· Commitment to practicing to the full scope of a pharmacist and lifelong learning through continuing education opportunities

Preferred Skills & Experience

· Hospital or primary care residency and/or work experience in a primary care/ambulatory care clinic setting

· Additional training or qualifications in advanced patient care (e.g., Certified Diabetes Educator, Certified Respiratory Educator, Certified Geriatric Pharmacist)

· Injection training and CPR certification

· Experience with Accuro EMR

· Experience in chronic disease management and patient education programming
