ONGOING CARE COACH (REMOTE)

REPORTING TO: Director of Centre Operations

Note: This is a part-time position; pay rate as agreed, based on experience, expertise, and applicable education. This is a remote position, however candidates should reside within a two-hour commute of one of our Teen Challenge Canada Centres.

COMPANY INTRODUCTION

Teen Challenge is a 12-month, faith-based, in-residence drug and alcohol rehabilitation program, for adults 18 and older. We currently have 8 centres located in Alberta, Atlantic Canada, Saskatchewan and Ontario that are able to serve up to 197 people who are struggling with substance addiction. We demonstrate God’s love by helping individuals become free from substance addiction and equipping them to live full and meaningful lives.

JOB POSITION DESCRIPTION

Our program is seeking a part-time Ongoing Care Coach (OCC) to support clients transitioning out of the Teen Challenge Canada program. The primary responsibility of the Ongoing Care Coach is to provide structured coaching and support to clients in the Transition phase, Phase 4, and post-graduation. You will work with clients to develop individualized care plans, support exit planning, and help them build the skills, confidence, and community connections necessary for long-term recovery. Your responsibilities will include: facilitating one-on-one coaching sessions, supporting clients through real-world challenges after graduation, connecting them with community resources, and collaborating with centre staff to ensure continuity of care. You will also be responsible for maintaining accurate client documentation, providing progress updates, and contributing to the ongoing development and improvement of the Ongoing Care program.
